READ CAREFULLY AND SIGN OR PLACE AN “X” IN THE BOX FOR ELECTRONIC SIGNATURE:
I hereby give permission for the following child(ren) to attend the Creation Evidence Expo Conference and to participate in the Creation Evidence Expo. I am aware that this is a Christian program that will expose my child to the teachings of the Holy Bible and scientific information. I do not object to the instructors explaining the Holy Bible, the Gospel of Jesus Christ, and/or leading my child(ren) to Him as their personal Savior.
LIABILITY RELEASE: In consideration of the Creation Evidence Expo allowing the child(ren) to participate, I, the undersigned, do hereby release, forever discharge, and agree to hold harmless the said organization, its directors, officers, presenters, instructors, and volunteers, from any and all liability, claims, demands, or causes of action for personal injury, sickness, or any injury as well as property damage and expenses, of any nature whatsoever, which may be incurred by the undersigned and the child(ren) above listed. Furthermore, on behalf of the children listed, I hereby assume all risks of accidental personal injury, sickness, or other damages and expenses as a result of participation in the Creation Evidence Expo and/or any program of Creation Evidence Expo, which may also include recreation and physical activities. The undersigned further agrees to hold harmless and indemnify Creation Evidence Expo, Charity Church, and Zion Unity Baptist Church, their directors, officers, presenters, instructors, pastors, and all workers from all liability as a result of the negligent, willful or intentional acts of the child(ren) or other participants, including expenses incurred attendant thereto.
